Daily Huddle Template for Clinics: Agenda, Huddle Board Ideas and Follow-Up

Below is a free one-page daily huddle template for outpatient clinics and practice groups. It has a timed agenda, a huddle board layout and an action-item table, so every issue raised leaves the huddle with an owner, a due date and a place to follow it up. The template covers the operational and administrative huddle: staffing, schedule flow, supplies, facilities, IT and announcements. It runs alongside your clinical systems and protocols. It is not a clinical handoff or safety-alerting tool.

Action items

Issue Owner Due date Status (open / in progress / done / blocked)

Most huddle templates stop at the agenda. The action-item table is what keeps the same problem from coming back every morning. If an item has no owner and no due date, it hasn't been resolved. It has only been mentioned.

The template has operational fields only. There are no fields for patient names, dates of birth, appointment details or other protected health information (PHI). Keep it that way when you adapt it.

What is a huddle in healthcare?

A huddle is a short, standing, recurring team check-in at the start of a day or shift. The team aligns on what matters today. It is not a staff meeting. Nobody sits down, nothing gets debated at length, and the goal is to surface what is different or at risk today so someone owns it.

Common types:

  • Daily operational huddle. Covers staffing, schedule flow, supplies, facilities, IT and announcements. This is the huddle this page is built for.
  • Safety huddle. Focuses on patient safety concerns and follows the organization's clinical protocols.
  • Department or location huddle. A huddle scoped to one team, such as front desk or billing, or to one site in a multi-location group.

This page covers the operational and administrative huddle for clinic and practice staff. Clinical safety huddles follow your organization's own clinical protocols and systems.

In an outpatient clinic, the morning huddle usually includes:

  • Practice or clinic manager
  • Front desk lead
  • MA or nursing lead
  • Billing representative
  • A provider representative, if the practice chooses

How to run a daily huddle in 7 steps

  1. Same time, same place, strict time box. Pick a spot near the front desk or break room and a time before the first patients arrive. Remote sites use the same recurring call. Everyone stands, and the lead watches the clock.
  2. Rotate the huddle lead. A weekly rotation spreads ownership and keeps the huddle from becoming the manager's monologue. The front desk lead, MA lead and billing lead can all run it.
  3. Update the board before the huddle, not during it. The board owner adds new cards and moves finished ones 10 minutes before start. Huddle time is for decisions, not for writing.
  4. Review yesterday's action items first. For each item: closed, still open or blocked. Blocked items get a new next step or move to the manager's list.
  5. Report exceptions only. "Front desk is normal" is enough. Speak up for what's different: "Dr. A's afternoon is double-booked from 2 to 4," "The autoclave is down," "Two new hires start Monday."
  6. Park long discussions. If something takes more than a minute, name an owner to set up a follow-up and move on.
  7. Read back new action items. End by reading each item aloud with owner and due date, then post the outcomes where absent staff will see them.

Huddle board ideas for clinics

A simple layout that works on a whiteboard or corkboard:

Today Open items Wins
Today's exceptions: schedule gaps, coverage, closures Action items with owner and due date Recognition and resolved issues

Add two strips:

  • Staffing and coverage strip across the top: who is in, who is out, who is floating, and which roles are uncovered.
  • Announcements corner: policy reminders, training deadlines, inspections and visitors.

Ideas by area

  • Front desk: open slots to fill, phone backlog, check-in delays, a new insurance verification step. Pair this with your front desk training checklist when new receptionists start.
  • Operations: supply reorders placed or pending, equipment out of service with an expected fix date, vendor visits. In a dental practice, the huddle fits naturally into the dental office manager's duties and daily checklist.
  • HR: onboarding start dates, training or certification renewals due this month, open shifts to cover. Managers tracking their own credentials can compare medical and dental office manager certifications.
  • IT: known system issues, planned downtime windows, password reset reminders, new device setup.

Make status visible. Use three status markers (open, in progress, done) and write a date on every card. A card dated nine days ago stands out on its own and prompts the question "who owns this?"

Keep a recognition section. One specific thank-you per huddle is enough: "Maria covered check-in and phones solo during the 8 a.m. rush." For more, see these staff appreciation ideas.

Privacy note: keep patient-identifying information off physical boards and out of photos of boards, including photos shared in group chats. Use operational descriptions such as "2 p.m. block overbooked" instead of patient details.

Physical board vs. digital huddle board for multi-location practices

A whiteboard works when everyone is in one room at 7:45 a.m. It misses part-time staff, the afternoon shift, people on PTO and every other location.

Physical board Digital board (channel, pins, files, tasks)
Reach One room, one shift All shifts and locations
Late arrivals and part-time staff Must read it in person Read it on phone or desktop
Remote sites Not included Join by call in the same workspace
History Erased daily Searchable
Upkeep Needs a board owner Needs a board owner

A digital huddle board doesn't need special software. A workable setup:

  • One huddle channel per location, such as "#huddle-north-clinic."
  • The agenda template pinned in the channel or stored in shared files.
  • Announcements pinned so they stay at the top.
  • Remote sites joining by audio or video call inside the same workspace instead of a separate meeting tool.

The tradeoff is the same as with a whiteboard. Someone has to own the board and keep it current. Name that person on the template.

Turning huddle items into tracked follow-ups with Chanty

Chanty is staff chat and work coordination for clinics and practice groups. It is a practical home for the digital version of your huddle, built from channels, pinned messages, files and tasks.

  • Message to task in one click. When someone posts "Exam room 3 printer jammed again" in the huddle channel, turn that message into a task. Give it an owner and a due date, and track it on the built-in Kanban board.
  • Start tomorrow from today's open items. Teambook gathers tasks, files, links and pinned messages in one place. The next huddle lead opens it and reads yesterday's open items without reconstructing them from memory.
  • Reach staff who missed it. Post the huddle outcomes and pin them in the channel. Scheduled posts and polls are available on Business and Enterprise.
  • Include remote sites. Unlimited 1:1 calls are included on Free. Unlimited group calls and screen sharing are on Business.
  • Reach the front desk and deskless staff. Apps for iOS, Android, macOS, Windows and web, with a messenger-style interface that needs little training.

Limits, stated plainly: task management in Chanty is basic, using list and Kanban views. It is fine for huddle follow-ups but not for complex projects. Chanty has no built-in shift scheduling, time tracking or HR functions. It runs alongside your EHR, scheduling and workforce management systems and does not replace them.

See how it fits your practice on our page about team communication software for clinics, or start free at chanty.com/signup.

Keeping the huddle channel appropriate for healthcare

Default rule: huddle notes stay operational and leave out patient details. "Afternoon schedule is overbooked" belongs in the huddle channel. Patient names and clinical details do not.

If your team does need to share PHI in staff chat, Chanty supports HIPAA-compliant chat for staff with a signed Business Associate Agreement (BAA) on the Business and Enterprise plans. The Free plan has no BAA and should not hold PHI.

The BAA is one part of a shared responsibility. Your practice is responsible for:

  • Signing the BAA before any PHI is shared
  • Configuring roles and permissions
  • Managing user access and offboarding departing staff
  • Device security
  • Staff policies and training on what belongs in chat

Third-party integrations fall outside Chanty's BAA scope, so don't route PHI through connected tools. For details on encryption, controls and the BAA, see Chanty security, HIPAA and BAA.

Common huddle mistakes and fixes

Mistake Fix
The huddle turns into a 30-minute meeting Enforce the time box. Park anything over a minute with a named owner.
The same issues come up every day They were never given an owner or due date. Use the action-item table and review it first.
Only one shift or location hears it Post outcomes to the huddle channel right after and pin anything that must stick.
The board goes stale Date every card, name a board owner and clear done items daily.
The huddle becomes a status report from every person Report exceptions only. "Normal" is a complete answer.

The huddle opens the day. The end-of-shift operational handoff closes it, so the next team inherits open items instead of surprises.

Where Chanty fits. chanty.health is published by Chanty, a team chat for clinic staff with a signed BAA from the Business plan. If your team still coordinates in personal group chats, see how HIPAA-compliant staff chat works or start free.

FAQ

What is a daily huddle in healthcare?

A daily huddle is a short, standing team check-in at the start of the day or shift. The team aligns on staffing, schedule flow, supplies, issues and announcements, and assigns an owner to anything that needs action.

How long should a daily huddle be in a clinic?

Keep it short and time-boxed, with a fixed length your team agrees on and sticks to every day. If a topic needs more time, park it and schedule a follow-up with the people involved.

What should be on a healthcare huddle board?

A useful board has today's exceptions, open action items with owners and due dates, a wins and recognition column, a staffing and coverage strip, and an announcements corner. Keep patient-identifying information off the board.

What is the difference between a safety huddle and an operational huddle?

An operational huddle covers staffing, schedule, supplies, facilities, IT and announcements. A safety huddle focuses on patient safety concerns and follows the organization's clinical protocols. This template is for the operational huddle.

Who should attend a clinic's morning huddle?

Usually the practice or clinic manager, front desk lead, MA or nursing lead and a billing representative, plus a provider representative if the practice chooses.

Can we run a huddle across multiple locations or remotely?

Yes. Use one huddle channel per location with the agenda pinned. Remote sites join by audio or video call in the same workspace. In Chanty, group calls are on the Business plan.

Is it OK to discuss patient information in a huddle chat channel?

Keep huddle channels operational by default. If staff must share PHI in chat, do it only in a tool covered by a signed BAA, configured according to your practice's policies. In Chanty, that means the Business or Enterprise plan. The Free plan has no BAA and should not hold PHI.

Is the daily huddle template free and printable?

Yes. The template is free, available as a printable PDF and a copyable version, and contains operational fields only, with no fields for PHI.
