In a solo private practice, one clinician sees clients and handles most of the admin. In a group practice, several clinicians share a practice name, systems, admin staff and often an office, so work starts passing between people. Clinical records stay in your EHR. The new problem is the daily operational traffic between the owner, clinicians, front desk, billing and contractors. Chanty puts that traffic in one place: staff chat, announcements, assigned tasks, files and calls, with a signed BAA available from the Business plan. It is not a client messaging tool, a teletherapy platform or a place for therapy notes.

Private practice vs group practice: what actually changes
The short answer: a solo private practice is one clinician who delivers care and runs most of the business. A group practice is several clinicians working under one practice, sharing a brand, systems, administrative staff and often locations.
The structural differences matter, but they depend on your state, payers and goals. Review each of these with a healthcare attorney or CPA before you act:
- Entity type and how ownership is set up
- Clinician classification, W-2 employees vs 1099 contractors
- Group NPI and credentialing, including whether clinicians are credentialed under the group with each payer
- Payer contracts and supervision rules for pre-licensed clinicians
This page doesn't give legal, tax or licensing advice. It covers the part that catches most owners off guard: coordination.
In a solo practice, the to-do list lives in one head. In a group, new roles appear: a practice manager, an intake or front desk coordinator, a biller (in-house or outsourced). Every task now has a handoff, and every handoff is a chance for something to go missing.
| Solo private practice | Group practice | |
|---|---|---|
| Who answers the phone | The clinician, between sessions | A front desk or intake coordinator |
| Who verifies insurance | The clinician or an outsourced biller | An intake coordinator or biller, on behalf of several clinicians |
| How schedule changes are communicated | The clinician updates their own calendar | Changes must reach the front desk, other clinicians and room bookings |
| Where policies live | In the owner's head or a single document | A shared, current location every staff member can find |
| How new clinicians are onboarded | Not applicable | A repeatable checklist with owners and deadlines |
Where coordination breaks as a therapy group grows
Clinicians can't take calls. Back-to-back 50-minute sessions mean the front desk can't reach a therapist until the top of the hour. Messages pile up in personal texts, email, voicemail and sticky notes on the monitor.
Personal messengers fill the gap. A group text or WhatsApp group appears because it's easy. It also mixes work with private life, gives the practice no admin control over who's in the conversation, and isn't covered by a BAA the practice has signed. If you're weighing options, see how team chat tools compare on HIPAA.
Spreadsheets track follow-ups, but nobody owns each row. Insurance verifications, missing intake paperwork and room bookings sit in a shared sheet. A row marked "pending" for a week has no owner and no due date.
Part-time staff and second offices miss updates. A contract clinician who works Tuesdays and Thursdays doesn't hear that the office closes early Friday or that the intake process changed.
Offboarding is risky. When a clinician leaves, work conversations stay on their personal phone, and the practice has no way to remove them.

How Chanty keeps a therapy group's staff work in one place
Chanty is internal communication for your staff, used alongside your EHR, scheduling and billing systems. See the full list of Chanty features for healthcare teams.
- Team chats. Public and private conversations by role or location, such as front desk, clinicians, billing or Office 2. Add one-on-one messages, threads and mentions. Message history is unlimited and searchable, including on the Free plan.
- Announcements. Team channels and pinned messages for policy changes, office closures and schedule updates. On Business, you also get company-wide announcements, scheduled posts and polls.
- Assigned tasks. Turn any message into a task with an owner, due date, status and priority, then track it on a Kanban board.
- Files and Teambook. Keep shared procedures, onboarding checklists and internal forms in the workspace. Teambook gathers tasks, files, links, pinned messages and conversations in one hub.
- Voice notes and calls. A clinician can send a 20-second voice note between sessions instead of typing. Staff use audio and video calls for internal meetings, not for client sessions. The Free plan includes unlimited 1:1 calls, and Business adds unlimited group calls and screen sharing.
- Apps everywhere. Chanty runs on iOS, Android, macOS, Windows and web. The messenger-style interface needs little training, which matters when your staff are moving off a group text.
Example workflows for a therapy group practice
More process guides are in the operations library for practice managers.
New client paperwork and insurance verification. The intake coordinator posts an admin request in the billing channel: "New client starting Thursday with Dr. Patel. Insurance verification needed." They turn it into a task assigned to the biller, due Wednesday. The biller marks it done. Clinical details and the intake assessment stay in the EHR.
Schedule and room changes. A clinician's afternoon moves to telehealth in your teletherapy platform, which frees Room 3. The coordinator posts the change in the Office 1 channel. Office-wide changes, like a closure for a holiday, get pinned so part-time staff see them at their next login.
Onboarding a new clinician. The practice manager creates a set of tasks: credentialing paperwork, EHR and email accounts, policy read-through, first-week shadowing. Each task has an owner and a deadline. Procedures and the staff handbook sit in shared files the new hire can find on day one.
Internal requests to IT or HR. Create one channel per request type, such as #it-requests and #hr-requests. Each request ("Laptop in Room 2 won't connect to Wi-Fi") becomes a tracked task, so nothing depends on someone remembering a hallway conversation.
Monthly team meeting. Run it as a group video call on Business. Afterward, the practice manager posts the agenda and turns each follow-up into an assigned task.
HIPAA, the BAA and what stays out of chat
HIPAA compliance with a signed BAA is available on the Business and Enterprise plans. The Free plan has no BAA and should not be used for PHI. More detail is on HIPAA-compliant chat for staff.
Chanty supports your compliance program. It does not make a practice HIPAA compliant on its own. Under shared responsibility, your practice:
- signs the BAA before any PHI enters the workspace
- configures roles and permissions
- manages user access and offboarding
- secures staff devices
- writes staff policies and trains staff on them
Write a staff chat policy. Spell out what belongs in chat and what doesn't. Use the minimum necessary information for operational coordination. "New client Thursday 2 pm, needs insurance verified" is operational. Session content, diagnoses and treatment details are not chat material. Psychotherapy notes and clinical documentation belong in your EHR. If your practice provides substance use disorder treatment, or operates in a state with stricter mental health confidentiality rules, confirm what applies before setting your policy.
Security controls:
- Encryption in transit (TLS 1.2+) and encrypted file storage. Chanty is not end-to-end encrypted.
- On Business: 2FA, SSO and IP allowlisting.
- On Enterprise: SAML SSO, directory sync and SCIM.
What's outside the BAA:
- Third-party integrations such as Google Drive or Zapier are outside Chanty's BAA scope.
- AI agents are available only on Enterprise. Their BAA coverage for agent processing and external models is not documented, so don't let agents process PHI until coverage is confirmed in writing.
Setting up Chanty for a group practice
A suggested channel structure:
- #announcements (posting limited to the owner and practice manager)
- #front-desk
- #clinicians
- #billing
- One channel per location, such as #office-downtown
- Private channels for leadership and partners
Roles and permissions. On Business and Enterprise, 120+ permission settings control who can post announcements, create channels or invite guests. Business includes customizable roles. Enterprise adds unlimited custom roles.
Guest access. External parties, such as an outsourced billing service or a managed IT provider, can join as guests limited to selected conversations. Business includes 3 free guest seats per paid member. Confirm your practice has its own BAA with each vendor that may see PHI.
Rollout tips.
- Move one workflow off texting first. Schedule changes work well because everyone feels the pain.
- Run it in Chanty for two weeks.
- Announce a date to retire the old group chat, then retire it.
- Publish your staff chat policy at the same time. Start from the free HIPAA templates.
What Chanty is not
- Not an EHR or EMR. Session notes, progress notes and treatment plans don't go in Chanty.
- Not client messaging. There's no client portal, no client texting and no appointment reminders.
- Not a telehealth or teletherapy platform. Calls are for staff only.
- Not urgent clinical alerting. Don't use it for client safety or crisis escalations. Follow your existing clinical protocols.
- Not an HR or scheduling suite. There's no built-in shift scheduling, time tracking, payroll, credentialing or billing. Chanty runs alongside your scheduling, billing and EHR systems.
- Not a project management tool. Task management is intentionally basic: lists and Kanban.

FAQ
What is the difference between a private practice and a group practice in therapy?
A solo private practice is one clinician handling care and most admin. A group practice has several clinicians sharing a practice name, systems, admin staff and often locations. Operationally, the biggest change is that work moves between people instead of staying with one person.
When should a solo therapist consider becoming a group practice?
Common operational signals include:
- a waitlist you can't serve
- more admin hours than session hours
- referrals you turn away
- a need for coverage when you're out
Entity, tax, credentialing and licensing questions should go to an attorney or CPA.
Can therapists in a group practice text each other about clients?
Personal texting usually gives the practice no admin control and no BAA. A safer setup is a staff tool covered by a signed BAA, a written policy limiting chat to the minimum necessary operational details, and clinical content kept in the EHR.
Is Chanty HIPAA compliant for therapy practices?
HIPAA compliance with a signed BAA is available on the Business and Enterprise plans. Chanty supports your compliance program, but your practice remains responsible for configuration, access, policies and training.
Does the free plan include a BAA?
No. Use the Free plan only to try the interface, without PHI.
Can we use Chanty to message clients or run teletherapy sessions?
No. Chanty is for internal staff communication only. Use your dedicated client communication and teletherapy tools for those.
Where should psychotherapy notes live if we use a staff chat?
In your EHR, never in staff chat.
Can contract (1099) clinicians or an outside billing company use Chanty?
Yes. Contract clinicians can be workspace members with permissions suited to their role. Outside companies can join as guests limited to selected conversations. Confirm a BAA is in place with each vendor that may see PHI.
How do we remove a clinician's access when they leave the group?
Because conversations live in the practice's workspace rather than on personal phones, an admin removes the departing person's access. That step is your practice's responsibility. On Enterprise, directory sync and SCIM connect access to your identity provider.
Do Chanty's AI agents work with client information?
AI agents are available only on Enterprise. BAA coverage for agent processing and external models is not documented, so agents should not process PHI until coverage is confirmed in writing.
